Start an incident report

Make sure everyone is safe and follow your company’s accident process first. Then, when it’s safe, report it in the app.

  1. 1On Home, tap Report Incident. Or open Checks and tap Report an Incident.
  2. 2Confirm the Vehicle Involved, any Trailer Involved and the odometer reading.
  3. 3Tap Start Report and answer each question.
  4. 4Where an answer needs follow-up, add notes and any Supporting photos.
  5. 5Sign and tap Submit Report.

How it’s different from a check

Incident reports don’t use defect severity. Answers that need action become follow-up items for the depot team, marked FOLLOW-UP REQUIRED on the review screen. Photos are optional but help a lot.

Your company is emailed as soon as an incident report arrives, if they’ve set up incident alerts.

Still stuck?

Drivers: ask your fleet manager first, they can fix most access problems. Managers: sign in and open a ticket, or use the contact form if you can’t sign in.
